Step 1: Define M&E for the program you actually run
Before choosing a framework, name what you are measuring and why. Three decisions set the ceiling on everything downstream: the unit of analysis (individual participant, household, community, organization), the change horizon (during implementation, at exit, at 6-month follow-up, at 2-year follow-up), and the decision the evidence is meant to inform (funder reporting, program redesign, both). When these three decisions are ambiguous, every indicator added later compounds the ambiguity — and the Graveyard grows.

Step 2: Build your M&E framework
An M&E framework is the map from activities to impact. It defines what to measure at each level of the results chain, which indicators track progress, how data will be collected, and how findings feed back into the program. Four framework types dominate the sector — and the choice between them shapes everything that follows.
The Logical Framework (Logframe) is the most widely used structure in international development. It organizes a program across four levels — Inputs, Activities, Outputs, Outcomes — with Assumptions running alongside each level and Indicators, Means of Verification, and Targets filling the matrix. Most bilateral donors require a logframe. The weakness: it can become a compliance artifact that gets filed after the proposal and never opened again. The Results Framework (used by USAID and many foundations) organizes around a goal, sub-goals, and intermediate results, with indicators assigned to each node. Results frameworks are less prescriptive than logframes — they describe what success looks like without specifying how to get there, giving implementing organizations more design latitude and often producing sharper indicator sets.
Theory of Change maps the causal logic from activities to long-term change and surfaces assumptions at each step. It is most useful as a design and learning tool rather than a tracking matrix. Organizations that start with a strong theory of change and then derive their indicator framework from it tend to land on fewer, more meaningful indicators — which shrinks the Graveyard before it opens.
MEL Frameworks integrate the learning function explicitly: when findings will be reviewed, who will decide based on them, how programs will adapt. MEL frameworks are increasingly required by funders who have grown skeptical of M&E systems that collect data without demonstrating that it changed anything.
For most organizations, the framework choice is determined by funder requirements first, then organizational capacity. The structural choice matters less than whether the framework's indicators are actually connected to data collection instruments — which is the job of the M&E plan.
Step 3: Write your M&E plan
The framework defines what to measure. The plan specifies how measurement actually happens. This distinction is the single most common source of M&E failure: organizations approve ambitious frameworks, then write plans that never specify where the data will come from. A credible M&E plan contains seven components.
Indicator definitions. Each indicator needs a precise definition, unit of measurement, disaggregation requirements (gender, age, geography, program type), data source, collection method, collection frequency, responsible party, and target. Vague indicators like "improved wellbeing" produce vague data. Precise indicators like "percent of participants scoring ≥70 on the validated 10-item wellbeing scale at 3-month post-exit follow-up" produce usable evidence.
Baseline and target-setting. Without a baseline you cannot measure change — only level. Baselines require collecting the same indicators from the same population before the program starts, which requires knowing your measurement instruments before you begin. Many organizations skip this step because it feels bureaucratic. They regret it when a funder asks for outcome evidence.
Data collection instruments. For each indicator, which survey, form, or interface will capture the data? Are those instruments already designed and tested? Are they linked to participant records that persist across collection points? This is where the Graveyard is most visible — indicators that exist in the framework but have no corresponding instrument in the plan.
Roles and responsibilities. Who collects data, enters it, reviews it for quality, analyzes it, and uses findings? M&E plans that assign every role to "the M&E team" — a team of one, usually — are plans that will produce reports six months late.
Data management procedures. Where is data stored, how are participant records deduplicated, what is the version-control protocol for survey instruments, what happens when contact information changes? These questions look operational. They decide whether your data is usable for longitudinal analysis.
Analysis plan. Which methods, who conducts the analysis, at what frequency, with which software? The plan should match your organization's actual capacity — not an aspirational methodology copied from an academic evaluation.
Reporting and feedback loops. When will findings be shared, with whom, in what format? Which findings go back to program staff in time to influence implementation, not just to funders after implementation has ended? Plans that specify funder reporting but skip staff feedback produce accountability without learning.

Step 4: Design indicators that actually get fed
Every indicator in your framework should pass three questions before it stays in the plan. These three questions are the difference between a 12-indicator framework that produces evidence and a 40-indicator framework that produces a Graveyard.
Who will collect this data, how, and when? If the answer requires action that is not already scheduled in someone's work plan, the indicator is a liability. Simplify it, combine it with a neighboring indicator, or remove it. A framework with fewer indicators that each have a data pipeline will produce better evidence than a framework with many indicators that mostly don't.
How will this indicator connect to a participant record? Indicators that require longitudinal tracking — pre to post, intake to exit, program to follow-up — require persistent participant IDs assigned at first contact and carried through every subsequent touchpoint. Without them, you are comparing cohort averages, not measuring individual change, and you cannot answer funder questions about who benefited. A workforce development program reporting to a foundation might use four outcome indicators: percent of participants completing the full program (process); percent of completers placed in employment within 90 days of exit (outcome); median wage increase at 6-month follow-up relative to intake (outcome); and percent of participants reporting that they would recommend the program to a peer (experience). Each has a defined instrument, a defined collection window, a defined responsible party, and a defined target. That is a usable indicator set. A list of twenty outcome indicators with no instruments behind them is not.
A nonprofit delivering financial literacy to small-business owners might use five indicators: percent of participants scoring ≥70 on the financial literacy assessment at exit (knowledge); percent reporting new financial practices adopted at 3-month follow-up (behavior); percent reporting increased business revenue at 6-month follow-up (outcome); percent of reported revenue increase attributable to program practices (attribution, open-ended then AI-coded); and percent recommending the program (experience). Step 5: Connect M&E to implementation and handoff to learning
Most M&E data collection happens in three windows: baseline before the program, midline during, endline after. This structure is necessary for evaluation. It creates a monitoring gap: nothing between baseline and midline, and nothing useful emerging until endline analysis is complete — often months after the program has ended. Closing this gap requires three infrastructure choices.
Participant records must persist across every data collection point. Intake forms, session logs, mid-program surveys, exit assessments, and follow-ups all need to link to the same individual record automatically.
