Scholarship Application Template: From Submission to Recipient Follow-Up

Download an editable scholarship application field template. Separate completeness, eligibility, human review, offer acceptance and recipient follow-up.

Build the application around your published criteria

A scholarship application should collect the evidence reviewers need for the stated award. Every question should support eligibility verification, a published selection criterion or administration of the application. Avoid collecting unrelated sensitive information because a generic template happens to include it.

This editable field map includes applicant questions and clearly separated staff-only fields. It does not contain legal terms, a universal eligibility policy or an automated award rule.

Use five distinct stages

1. Application

Identify the award, cycle, applicant, goals, plan and required evidence.

2. Completeness

Request missing evidence. Incomplete does not automatically mean ineligible.

3. Human review

Apply the published rubric version and retain evidence and reviewer reasoning.

4. Offer and acceptance

Record the offer separately from the recipient’s acceptance.

5. Follow-up: collect the recipient update for the agreed period. Keep award administration and payment controls with the appropriate verified process.

Example: a missing statement is a follow-up task

Synthetic example: an applicant supplies a plan and budget but omits the required supporting statement. Staff record “incomplete—statement requested.” They do not assign a zero for motivation or infer that the applicant is unsuitable. After receiving the statement, reviewers evaluate the complete evidence against the published criteria.

AI can help organize and interpret supplied evidence against configured instructions. Keep its suggestions separate from the final human decision. Requests for accessibility support or help completing a form should not automatically reduce an applicant’s score.

Before you open applications

  • Confirm the award, dates, prerequisites and rubric version.
  • Identify which fields are applicant-facing and which are staff-only.
  • Test a complete, incomplete and borderline example.
  • Check reviewer and applicant access with separate accounts.
  • Define correction, decision and acceptance procedures.
  • Confirm the responsible person for recipient follow-up.
